We invite you to participate in a research study being conducted by investigators from The University of Iowa and the National Association of School Nurses. The purpose of the study is to describe the current practices of school nurses in administering medications in schools.

If you agree to participate, you will be asked to complete an online survey accessed through the link provided about medication use in schools. You are free to skip any questions that you prefer not to answer. It will take approximately 15-30 minutes to complete the survey. We suggest you collect the data outlined in the accompanying email before you begin the survey, this will facilitate the time needed to complete the survey. If you are unable to complete the survey in one sitting click the "Save & Return Later" button to resume the survey at another time.

On completion of the survey, if you choose, you may provide your email address in order to be entered in a drawing for an IPad. Otherwise, we will not collect your name or any identifying information about you. It will not be possible to link you to your responses on the survey.
